Creatine+ Survey 1
We’re seeking 30 participants, age 21+, to join a 30-day survey for our new Qualia Creatine+.
Participants will receive a free 30-day supply of Creatine+ and take one scoop daily, mixed in water or a beverage of choice, with or without food. In return, we’ll ask you to complete short surveys before starting and at key points throughout the study.
Your feedback will be used in our marketing materials, so we ask for an honest review.

Participation Requirements:
- Healthy adults 21 years or older
- Can read and write English
- Currently not taking any supplements that contain Creatine
- Willing to not begin taking any new supplements during the study and continue taking any supplements they are currently using regularly.
- Willing to complete questionnaires, records, and diaries associated with the study.
Exclusion Criteria:
- Women who are pregnant, breastfeeding or planning to become pregnant during the trial
- Known food intolerances/allergy to any ingredients in the product
- Having any of the following conditions: Psychiatric conditions, neurologic disorders, high blood pressure, heart conditions, endocrine disorders, or cancer
- Taking MAO inhibitors, SSRIs, or any other psychiatric or neurological medicines
- On immunosuppressive therapy
This survey is being conducted by Qualia Life Science
